Revolax Deep  is the medium strength grade in the Revolax system from Across Co., Ltd.: a cross-linked, monophasic hyaluronic acid filler with 24 mg/ml HA and 0.3% lidocaine, in a 1.1 ml pre-filled syringe with a 27G needle. It is intended for use in the mid to deep dermis and subcutaneous tissue – the layer used to fill nasolabial folds and contour cheeks, chin, or bridge of the nose.

Revolax is not a single injection, but a ladder of three strengths. They do not differ in their mechanism of action, but in their cross-linking density and therefore in the layer in which they are intended to be applied. Choosing the wrong strength will result in either nodules being applied too high or insufficient projection being applied too low.

Deep is the working grade with the broadest range of applications in most practices: firm enough for projection, yet fine enough for wrinkle correction. For superficial lines, we offer Revolax Fine , and for pronounced volume deficiencies , Revolax Sub-Q – all three with the same monophasic gel structure.

What “monophasic” practically means

Monophasic means a single, fully cross-linked gel instead of a suspension of gel particles in a carrier medium. The difference is evident in the syringe plunger.

With particulate (biphasic) fillers, particles are pushed through the cannula – the resistance fluctuates, and jerky exit can occur. A homogeneous gel flows more smoothly. For the practitioner, this primarily means control: the amount dispensed corresponds to the pressure applied, throughout the entire injection.

Revolax is produced from highly purified hyaluronic acid derived from bacterial fermentation – not of animal origin. According to the manufacturer, the cross-linking technology does not require the addition of free hyaluronic acid, which is used to adjust the flow properties of other fillers.

Lidocaine 0.3%: what the additive can do – and what it can't.

The lidocaine variant contains 0.3% lidocaine in the gel. This numbs the tissue during injection, making the treatment more comfortable for the patient. The additive does not replace nerve block anesthesia for particularly painful areas and only takes effect once the filler is being placed – you will feel the first injection site.

Two points that must be clarified before treatment: a known hypersensitivity to lidocaine or amide local anesthetics excludes its use, and the lidocaine variant also differs in HA concentration from the lidocaine-free version: 24 mg/ml instead of 20 mg/ml, with a fill volume of 1.1 ml instead of 1.0 ml.

Extrusion force: 18.5 N – and why consistency counts

The manufacturer specifies an extrusion force of 18.5 N for Revolax Deep, measured in their own comparative test against five competing products (values ​​there between 16.0 and 23.1 N).

What's interesting about this number isn't so much its magnitude – it's in the middle range – but its curve . In the manufacturer's extrusion diagram, the curve for Revolax Deep rises early and then remains flat, while other products show significant fluctuations along the injection path. This is precisely the point: A filler that offers constant resistance can be dosed precisely. One with fluctuating resistance forces constant readjustment of thumb pressure, leading to over- or under-corrections.

The values ​​are derived from manufacturer data ("Data on file, Across"), not from an independent comparative study. They are useful as a guide for handling; not as proof of clinical superiority.

Duration of effect: 8 to 12 months

The manufacturer states a duration of action of 8 to 12 months for all Revolax variants. The actual duration in each individual case depends on factors outside the product itself: injection depth, quantity, mobility of the treated area, metabolism, and lifestyle habits of the person being treated.

As a general rule, areas with high mechanical stress – lips, perioral area – deteriorate faster than less stressed areas like cheeks or chin. A follow-up appointment after two to four weeks is advisable to assess the result and, if necessary, perform further treatments.

Clinical data

A controlled study exists for Revolax – the precise classification is important here: Revolax Sub-Q with Lidocaine was studied , not the Deep variant.

The study design (ClinicalTrials.gov ID NCT02179606) was multicenter, randomized, participant- and investigator-blinded, split-face, comparative over 24 weeks. Sixty individuals with nasolabial folds of WSRS grade 3 or 4 were included. After 24 weeks, the mean WSRS score on the Revolax side was 2.14 compared to 2.49 on the control side; the overall GAIS assessment was 100% satisfactory for both investigators and participants, compared to 98% for the control product. The study was conducted at Chung-Ang University Hospital and Eulji General Hospital in South Korea. The authors concluded that Revolax was effective and safe for nasolabial fold correction and non-inferior to the reference product.

No independent efficacy claims can be derived for Revolax Deep from this study. We include it here because it belongs to the product family – not as evidence for this specific variant.

Handling, storage and goods receipt

Revolax Deep is supplied as a sterile pre-filled syringe. Before use, the sterile barrier and packaging are checked for integrity, as are the batch number and expiry date. The gel should be clear and free of visible particles. The injection is performed under aseptic conditions; the syringe is for single use only, and any remaining solution is discarded.

Unlike botulinum toxin, a hyaluronic acid filler does not require refrigeration. It should be stored at room temperature, protected from light and frost – freezing damages the gel structure. It has a shelf life of 24 months.
